Intermountain Healthcare and virtual specialty care company Story Health announced a strategic partnership to allow the health system’s patients with heart failure to access care outside the clinic.
Calif.-based Story Health offers telehealth, health coaches, AI and connected devices to treat patients with heart failure at home. It also lets doctors access patient data and receive personalized recommendations based on a physician’s historical prescribing patterns.
As part of the partnership, Intermountain clinicians and the health tech company’s coaches will collaborate to provide personalized treatment to patients and monitor medication access and adherence between visits, Mobi Health News reports.
The coaches will act as the go-between for patients and their providers, monitoring patient progress and answering questions via text, email or call. Read more.
