Behavioral healthcare startup Concert Health is partnering with Wisconsin-based healthcare provider Froedtert Health to enhance care for pediatric, primary care and women’s health patients.
Concert Health offers behavioral health support to primary care, women’s health and pediatric providers. Utilizing a collaborative care model, Concert uses referrals, prescription data and screening across a practice’s population to find patients who may benefit from behavioral health services. The company then connects them with a behavioral care manager through a phone or video call.
Managers can counsel patients, manage medications, check symptoms and help patients set goals. Psychiatric consultants can also review information about patients who are not improving and add recommendations to a primary care provider’s medical record.
Through the partnership launched in Spring, Concert Health’s behavioral healthcare services will be used by 30 providers in Wisconsin’s Manitowoc and Sheboygan counties, Mobi Health News reports. Read more.
