CMS is launching a new primary care model aimed at reducing costs and provide integrated care, per Becker’s.
The “Making Care Primary” innovation will provide a pathway for physicians to gradually adopt population-based payments, supporting the delivery of advanced primary care and improving patient outcomes, CMS said.
“MCP’s graduated care delivery, quality, and payment structures will provide an on-ramp for organizations looking to begin their transition to value-based care and will support them as they build infrastructure and advance in delivering accountable care,” the report said
The new model will particularly focus on primary care clinicians in rural and underserved communities and is set to run from July 2024 to December 2034.
