Founded in 2020, CRC provides Physiatry (the medical specialty of Physical Medicine and Rehabilitation), chronic care management, and behavioral health services to skilled nursing facilities nationwide to enhance clinical care and improve patient outcomes.

The round was led by Boston-based RA Capital Management with participation from existing investors Floating Point, also of Boston, and New York-based Courtside Ventures. Another new investor was Century City-based Time BioVentures.

The raise was led by Blackbird Ventures – with participation by Hostplus, Hesta, Wormhole Capital, Archangel Ventures, Possible Ventures and Saniel Ventures.

Spark Capital led the funding round and was backed by existing investor Bessemer Venture Partners.

Ascend represents the combination of five plastic surgery practices and two standalone MedSpas.

The amount of investment was not disclosed.

The investment will back companies that can create better quality of life, offer resources to entrepreneurs with disabilities and provide support to people with disabilities who are unemployed or underemployed.

Maveron led, and was joined by all major existing investors.

Flare Capital Partners led the round.

CVS Health Ventures led the round, in conjunction with LRVHealth, Ascension Investment Management (a subsidiary of Ascension), and the University of Wisconsin Foundation.
